Most shoulder pain responds best to a progressive loading program rather than rest. We build that at four Suffolk County offices, with hands-on work and needling as additions where they help.
If you recognized yourself in more than one of those, that is enough to book. You do not need a diagnosis to come in, and you do not need a referral to start.
Most people do best on a combination. Your first visit sorts out which of these you actually need, and in what order.
This carries the guideline's strongest recommendation for shoulder pain. A progressive loading program is the treatment, not the warm-up to it.
The 2025 guideline says clinicians may add acupuncture alongside active rehabilitation, on weaker evidence. Alongside, not instead.
Manual therapy is supported for short-term pain relief, which is often what makes the loading program possible in the first weeks.

We work out the cause
A full history and a hands-on exam. We look at how you move, not only where it hurts, so the plan targets the thing driving the pain.

Treatment starts the same day
You are treated on the first visit in most cases. No waiting for a second appointment to begin.

We reassess as you go
We check progress at set points and adjust. If something is not working we change it or tell you honestly that we are not the right fit.
We accept The Empire Plan/NYSHIP, BlueCross BlueShield, Aetna, Cigna, Medicare and UnitedHealthcare, and other plans that have out-of-network coverage. We check your benefits before your first visit and tell you what you will owe, so nothing lands on you at the front desk.
We handle the billing. In most cases you pay a co-insurance amount at the visit and we take it from there.
Not every plan covers every service, and coverage depends on medical necessity. A few plans place us outside their network. When that happens we still file the claim for you and give you the cost in writing before you book.

Rotator cuff related shoulder pain usually responds best to a progressive loading program. The 2025 physical therapy clinical practice guideline gives its strongest recommendation to active rehabilitation exercise, supports manual therapy for short-term pain relief, and says clinicians may add acupuncture alongside active rehabilitation based on weaker evidence. Cochrane found manual therapy plus exercise improved function only slightly more than placebo. So we lead with loading exercise and use the rest as additions.

A sudden inability to lift the arm after an injury may be an acute full-thickness tear and needs a surgical opinion. Shoulder pain with fever or feeling unwell, night pain with unexplained weight loss, or shoulder and arm pain with chest tightness, sweating or breathlessness needs urgent medical assessment rather than an appointment here.

Usually not. Many rotator cuff problems do as well with rehabilitation as with surgery, and we will tell you if we think you need a surgical opinion.
Not necessarily, and we will not sell you volume. The evidence suggests a well-taught home program does much of the work.
No. It can reduce pain and improve how the shoulder works, which is a different and still worthwhile thing.
Usually months rather than weeks for a loading program to change what the shoulder tolerates. We will be honest about that up front.
